摘要
回顾性分析2021年5月-2024年3月该院收治的29例合并高危因素的肾和输尿管上段结石患者的临床资料。其中,合并心功能不全6例,肺功能不全10例,肾功能不全10例,高血压10例,糖尿病5例,喉癌术后困难气道1例,脑血管及神经病变7例。采用局部麻醉后,在超声引导下行16 F微通道PCNL,术后常规放置输尿管内支架管,不留置肾造瘘管,即:部分无管化PCNL。
所有患者均顺利完成手术,6例患者因感染和肾功能损害明显,先行局部麻醉下超微造瘘(F 6单J管)引流,病情稳定后行PCNL。手术时间为30~94 min,平均(55.8±16.7)min,术中疼痛视觉模拟评分法(VAS)3~6分,平均(4.2±0.9)分,术后血红蛋白下降1~26 g/L,平均(8.4±6.6)g/L,术后疼痛VAS 2~7分,平均(3.2±1.2)分,术后2至3天复查腹部CT,结石清除率为82.8%(24/29),1例术后3周行局部麻醉下输尿管软镜清石,2例行体外冲击波碎石排出,2例因多发小结石行药物排石治疗。1例术后因动静脉瘘行输血和肾动脉介入治疗,无中转开放病例,无胸膜损伤等并发症发生。

回顾性分析2021年5月-2024年3月本院收治的29例合并高危因素的肾和输尿管上段结石患者的临床资料。所有患者术前通过泌尿系彩超、腹部CT和静脉肾盂造影明确结石诊断。其中,男19例,女10例,年龄35~91岁,平均(64.7±14.2)岁;体重指数19.1~27.0 kg/
纳入标准:肾单发结石最大径≥2.0 cm;输尿管L4平面以上结石,最大径≥1.5 cm;肾多发结石或部分鹿角形结石;ASA分级为Ⅱ~Ⅳ级。排除标准:不能耐受俯卧位者;凝血功能障碍不能纠正者;严重的重要脏器功能障碍者。
术前嘱患者行俯卧位30 min,预估患者对手术体位耐受情况,并由主刀医生联合麻醉医师共同与患者和家属沟通病情和治疗方案,充分告知患者手术和麻醉风险。术中进行全程麻醉监护,根据术中情况,可适当加镇静和止痛药物,必要时中止手术或更换为全身麻醉或硬膜外麻醉。
患者术前15 min常规使用盐酸哌替啶50 mg+盐酸异丙嗪25 mg肌肉注射。先行截石位,常规消毒铺巾后,使用盐酸丁卡因胶浆(利宁)对尿道进行表面麻醉并适当润滑,输尿管镜直视下逆行置入5 F或6 F输尿管导管至结石下方或肾盂出口处,置入尿管并固定,输尿管导管外接冲洗水备用。更换俯卧位,脐下垫软枕适当抬高腰部,有利于减少术中肾脏活动度。结合术前腹部超声、CT和静脉肾盂造影,先行超声,评估结石和积水情况,确定穿刺点、穿刺路径和目标盏。再次消毒、铺巾,使用2.0%利多卡因10 mL+0.9%氯化钠液5 mL稀释备用,以目标肾盏穹隆部穿刺点为中心,用5 mL一次性注射器针头,对中心周围直径约3.0 cm范围内的皮肤、皮下组织和肌肉行全层浸润麻醉。注射过快,会导致穿刺路径区域出现液性或气性团块区域,要注意避免后续超声伪影形成,以防影响穿刺通道的建立及术中结石定位的评估。麻醉满意后,再次超声定位,穿刺针沿穿刺点向目标盏穹窿处缓慢进针至肾周筋膜区域,再次确认穿刺针尖端和目标盏,嘱患者适当减慢呼吸,继续进针至肾盏内,拔除针芯,可见淡黄色或淡红色尿液流出,则提示穿刺成功。继续在超声引导下将导丝沿肾盏—肾盂方向置入,沿穿刺针切开皮肤和皮下组织,确定穿刺深度后,退出穿刺针鞘。采用两步扩张法,即先8 F后16 F,建立16 F并留置Peel-Away鞘作为操作通道。使用科医人60 W钬激光击碎结石,自工作鞘冲出结石,钬激光工作参数:输尿管上段嵌顿结石脉冲能量1.0~1.5 J,脉冲频率20 Hz;肾结石脉冲能量2.0~2.5 J,脉冲频率20 Hz。如术中发现肾积脓,彩超见残石,或手术结束检查穿刺通道见出血,则予以留置14 F双腔气囊造瘘管,气囊注入0.9%氯化钠液2 mL固定,其余情况术后不留置肾造瘘管。
所有手术均采用单通道PCNL顺利完成,术后常规留置4.8 F输尿管支架管,均未留置肾造瘘管。手术时间为30~94 min,平均(55.8±16.7)min,术中疼痛VAS 3~6分,平均(4.2±0.9)分,术后血红蛋白下降1~26 g/L,平均(8.4±6.6)g/L,术后疼痛VAS 2~7分,平均(3.2±1.2)分,术后住院时间2~7 d,平均(3.2±1.3)d,手术结石清除率82.8%(24/29),1例术后3周行局部麻醉下输尿管软镜清石,2例行体外冲击波碎石排出,2例因多发小结石行药物排石治疗。1例术后因动静脉瘘行输血和肾动脉介入治疗,无中转开放病例,无胸膜损伤等并发症。
PCNL作为治疗上尿路结石的主要手术方式之一,经过数十年的发展,已经十分成熟,且被各级医疗机构广泛应用,但仍存在学习曲线长和并发症多等问

本研究存在一定的局限性,属于回顾性研究,且样本量较少,缺乏对照组,仍需开展大样本量和多中心的前瞻性研究,进一步验证其安全性和有效性,并评估手术适应证,便于后续该方案的临床推广及应用。
综上所述,局部麻醉超声引导下微通道部分无管化PCNL治疗合并高危因素的上尿路结石,是一种安全、有效的手术方案,但由于该手术对手术医师要求较高,需要丰富的手术经验,还需密切关注患者围手术期病情变化。因此,术前应结合患者病情,与患方充分沟通手术和麻醉方式,术中严密关注患者病情变化,必要时辅以用药,或更改手术和麻醉方式。
