摘要
选取2019年6月-2022年6月该院因极外侧型椎间盘突出行UBE下椎间盘髓核摘除术治疗的患者25例。其中,男16例,女9例;年龄26~62岁,平均(53.67±17.45)岁。病史3 d~10年。椎间孔内型(I型)9例,椎间孔外型(Ⅱ型)13例,混合型(Ⅲ型)3例。L4/5间隙8例,L5/S1间隙17例。所有患者术前均行腰椎正侧位X线、CT和MRI检查。采用视觉模拟评分法(VAS)和Oswestry功能障碍指数(ODI)分别对手术前、术后第3天和术后3个月下肢、腰部疼痛和机体功能恢复情况进行评估,采用Macnab标准对近期手术疗效进行评价。
UBE技术治疗ELLDH,手术时间(79.79±23.97)min,术中出血量为40~80 mL,平均(55.80±10.74)mL。随访时间(7.02±4.26)个月。术前,下肢VAS为(7.04±0.92)分,腰部VAS为(3.49±1.52)分,ODI为(35.03±2.97)%,术后第3天,下肢VAS为(2.17±0.61)分,腰部VAS为(1.48±0.43)分,ODI为(18.77±3.15)%,术后3个月,下肢VAS为(1.38±0.65)分,腰部VAS为(1.03±0.48)分,ODI为(6.05±1.80)%,术后第3天和3个月较术前明显降低,差异均有统计学意义(P < 0.05)。优良率为96.0%(优20例,良4例,可1例),随访期间未发生并发症。
极外侧型腰椎间盘突出症(extreme lateral lumbar disc herniation,ELLDH)在1974年就有文
选取2019年6月-2022年6在浙江中医药大学附属两家医院接受诊疗的ELLDH患者25例,回顾性分析患者的临床资料。其中,男16例,女9例,年龄26~62岁,平均(53.67±17.45)岁;病史3 d~10年。根据MRI与CT扫描分
提前与麻醉医师沟通,行控制性降压,以减少术中出血,保持术野清晰,术中收缩压控制在100~110 mmHg,根据患者情况实时调控血压。选择可透视X射线的手术床、C臂机(图像清晰且方便调节)、30°关节镜、磨钻、椎板钳和髓核钳等。准备生理盐水作为灌洗液,术前铺巾时,预先铺设防水层,防止患者术中出现失温症。
全身麻醉后,将乳胶贴贴于患者双侧膝盖、髂前上棘和肋弓,以保护皮肤。置患者于俯卧位,使用半圆形体位垫,悬空腹部,以避免腹腔压力过大。以左侧L4/5间隙的ELLDH作为典型案例。将1根克氏针放置于C臂透视机接收器的中线处,并垂直地面,行侧位腰椎透视,调整角度体位,使L4/5椎间隙与水平面形成直角,打开椎间隙,然后行腰椎正位透视。在L4/5椎间隙体表投影处,放置克氏针,以确定责任间隙,在L4左侧椎弓根峡部为中心线的体表投影处,放置1根克氏针a,在L4/5左侧椎弓根外缘的体表投影处,放置克氏针b,在克氏针b外侧2.0 cm处,平行放置克氏针c,以a与c交点为中心,分别在头端和尾端距离1.5 cm处做手术切口,观察通道的位置位于头端,操作通道的位置位于尾端(

采用视觉模拟评分法(visual analogue scale,VAS),评估手术前、术后第3天和术后3个月的腰部及下肢疼痛程度,0分为无痛,10分为剧烈疼痛。采用Oswestry功能障碍指数(Oswestry disability index,ODI),对机体功能恢复情况进行评估,评估项目共计10项,最低得分0分,最高得分5分,计算方法:实际得分/50×100%,评估时间点为:手术前、术后第3天和术后3个月,得分越高,机体功能恢复越差。采用Macnab标准,评估术后近期疗效:优,无腰腿痛及活动受限,能正常生活及工作;良,偶尔有腰痛或腿痛,主要症状减轻,不影响工作和生活;可,一定程度的功能改善,但有阵发性疼痛,被迫改变工作和生活习惯;差,疼痛情况及功能状态无任何改善 。优良率为:优和良的例数占总例数的百分比。
手术时间(79.79±23.97)min,术中出血量为40~80 mL,平均(55.80±10.74)mL。术后第3天下肢VAS为(2.17±0.61)分,腰部VAS为(1.48±0.43)分,ODI为(18.77±3.15)%,术后3个月下肢VAS为(1.38±0.65)分,腰部VAS为(1.03±0.48)分,ODI为(6.05±1.80)%,较术前的下肢VAS(7.04±0.92)分、腰部VAS(3.49±1.52)分和ODI(35.03±2.97)%明显降低,差异均有统计学意义(P < 0.05)。见
