腹腔镜下保胆取石适应证文献检索分析

摘要:

目的 了解胆囊结石患者行保胆取石术，其适应证的执行情况。方法 以“腹腔镜”和“保胆取石”为主题词，检索CNKI和万方数据库2010年以来，单纯报道腹腔镜下保胆取石文献。根据文献中有无结石复发，分为复发组和无复发组，比较两者一般情况及适应证执行情况。结果 选取符合条件的文献35篇，复发组22篇共计2 176例患者复发52例，无复发组13篇共计620例患者；两组性别比差异无显著性，复发组和无复发组的年龄和随访时间分别为41.60、40.75岁和16.47、11.29个月，差异有显著性（P <0.05）；两组并发症数差异无显著性。两组患者适应证执行情况差异无显著性。结论 腹腔镜下保胆取石适应证的执行情况差异不大，术后结石复发可能与年龄和随访时间有关。

Abstract:

【Objective】 To investigate the indications of laparoscopic cholecystolithotomy with gallbladder preserved. 【Methods】 "Laparoscopic" and "cholecystolithotomy with gallbladder preserved" as the theme words, the literature in "CNKI" and "Wanfang database" were searched since in 2010, which reported cholecystolithotomy with gallbladder preserved. According to the literature whether the stone recurrence was divided into recurrence group and non recurrence group, analyzed the general conditions and indications of implementation. 【Results】 Selected qualified literature of 35 references, a total of 2176 patients in 22 articles as recurrent group, which had 52 cases recurrence; No recurrence group was 13 articles with 620 patients; The two groups had no significant difference in sex ratio, value of age and follow-up time of recurrence group and non recurrence group of were 41.60, 40.75 years and 16.47, 11.29 months respectively, there was a significant difference (P <0.05). The complications of two groups of patients had not significant difference. Two groups of patients with indications had no significant difference. 【Conclusion】 The indications of all the surgery of laparoscopic cholecystolithotomy with gallbladder preserved have no differences, postoperative recurrence may be related to age and follow-up time.
