超声内镜引导部分脾动脉栓塞术治疗门静脉高压脾功能亢进的临床疗效分析（附7例报告）

Analysis of the clinical efficacy of endoscopic ultrasound-guided partial splenic artery embolization for portal hypertension and hypersplenism (report of 7 cases）

摘要:

目的 探讨超声内镜引导部分脾动脉栓塞术（EUS-PSE）治疗门静脉高压脾功能亢进的临床疗效。方法 回顾性分析2025年3月1日－2025年8月31日于该院行EUS-PSE治疗的7例门静脉高压脾功能亢进患者的临床资料，评估治疗效果。结果 7例患者均顺利完成EUS-PSE。所有患者均采用弹簧圈联合三明治组织胶注射术（聚桂醇3.0 mL + 组织胶0.5 mL + 聚桂醇3.0 mL），进行脾动脉栓塞，6例患者血小板明显升高。所有患者均未发生穿孔、出血和异位栓塞，1例患者术后出现严重腹腔感染，经抗感染治疗1个月后好转。结论 EUS-PSE治疗门静脉高压脾功能亢进的临床疗效好，但需要密切关注脾周及腹腔感染情况。

Abstract:

Objective To explore the clinical efficacy of endoscopic ultrasound-guided partial splenic artery embolization (EUS-PSE) in the treatment of portal hypertension and hypersplenism.Methods A retrospective analysis was conducted on the clinical data of 7 patients with portal hypertension and hypersplenism who underwent EUS-PSE from March 1, 2023, to August 31 2025. The therapeutic effects were evaluated.Results All 7 patients successfully completed EUS-PSE. All patients underwent splenic artery embolization using a combination of coils and sandwich tissue glue injection (3.0 mL of lauromacrogol + 0.5 mL of tissue glue+3.0 mL of lauromacrogol). Platelet counts significantly increased in 6 patients. None of the patients experienced perforation, bleeding, or ectopic embolization. One patient developed severe abdominal infection after the operation, which improved after 1 month of anti-infection treatment.Conclusion EUS-PSE is effective in treating portal hypertension and hypersplenism. However, close attention should be paid to splenic and abdominal infections.
