新疆2 488例汉族和维族十二指肠溃疡内镜资料对比分析

Comparative analysis of endoscopic data of 2 488 Han and Uyghur patients with duodenal ulcer in Xinjiang

摘要:

比较新疆地区汉族和维族近6年的十二指肠溃疡检出率、性别、年龄、季节、病变部位和并发症等疾病特点，研究两民族十二指肠溃疡的发病趋势，为不同民族十二指肠溃疡防治工作的侧重点提供依据。方法 收集2013年1月－2018年12月在新疆克拉玛依市中心医院就诊并确诊为十二指肠溃疡的汉族和维族患者2 488例。其中，汉族2 266例，维族222例，各类数据使用SPSS 19.0软件进行统计学分析。结果 汉、维族十二指肠溃疡检出率差异有统计学意义（P <0.01），汉族十二指肠溃疡检出率呈下降趋势，降幅35.48%，而维族十二指肠溃疡检出率呈现下降后又回升态势，两民族幽门螺杆菌（Hp）检出率和十二指肠溃疡检出率趋势相符。两民族十二指肠溃疡检出率较高的年龄段均为26～35岁，46～55岁十二指肠溃疡患者占比均最高，维族此年龄段十二指肠溃疡检出率高于汉族（P <0.05）。2 488例中，56岁以上汉族十二指肠溃疡占比高于维族（P <0.01）。汉族十二指肠溃疡检出率冬季最高，维族则是在春季，其春季十二指肠溃疡检出率和发病占比均高于汉族（P <0.01，P <0.05）。两民族患者性别、十二指肠溃疡发病部位、并发症和多发溃疡率差异均无统计学意义（P >0.05），汉族复合溃疡率比维族高（P <0.05）。结论 两民族2013年1月－2018年12月上消化道内镜检查人数呈上升趋势，汉族十二指肠溃疡检出率持续降低，而维族十二指肠溃疡检出率已回升到较高水平，维族Hp检出率高于汉族。维族应比汉族在更年轻时、而汉族56岁以后应比维族更容易发生十二指肠溃疡。汉族在冬季、维族在春季十二指肠溃疡发病率最高。汉族复合溃疡率比维族高。

Abstract:

To compare the detection rate, gender, ages, season and lesion location and complication between Han and Uyghur with duodenal ulcer in Xinjiang, and investigate the trend of the two nationality’s duodenal ulcer in recent 6 years and provide the evidence of prevention and treat for the difficult ethnic. Methods Clinical data of 31 205 cases of duodenal ulcer (2 266 Han and 222 Uyghur) from January 2013 to December 2018 was collected, then statistical analyzed the data by SPSS 19.0. Results There were significant difference in the detection rate of duodenal ulcer between Han and Uyghur (P < 0.01). The detection rate of duodenal ulcer of Han were sustainable declined to below 35.48% in recent 6 years, and Uyghur’s rate of duodenal ulcer were raised again after fall. The trend of detection rate of Hp was corresponded with duodenal ulcer between two nationality. The detection rate of duodenal ulcer were all higher in 26 ~ 35 age groups and the proportion of duodenal ulcer were highest in the 46 ~ 55 age groups between the two nationality. Uyghur’s detection rate of duodenal ulcer was higher than Han in the 46 ~ 55 age groups (P < 0.05), the proportion of the elder patients (>56) of Han duodenal ulcer were higher than Uyghur (P < 0.01). The detection rate of duodenal ulcer of Han was highest in the winter, and then Uyghur was in the Spring, the detection rate and the proportion of duodenal ulcer of Uyghur was higher than Han in the Spring (P < 0.01, P < 0.05). There were no difference in the gender, lesion location, complication rates and multiple ulcer between Han and Uyghur’s duodenal ulcer patients (P > 0.05), the rate of complex ulcer of Han’s was higher than Uyghur (P < 0.05). Conclusion The two nationality number of endoscopic procedures increased in recent 6 years, the detection rate of duodenal ulcer of Han were sustainable declined, and then the detection rate of duodenal ulcer of Uyghur was raised the high level again. The detection rate of Hp of Han was lower than Uyghur. Uyghur should be attention to happen with duodenal ulcer than Han in younger age, then Han should be attention to happen with duodenal ulcer after 56 years old. Uyghur’s the top season of detection rate of duodenal ulcer was winter and Han was Spring. The rate of complex ulcer of Han’s was higher than Uyghur.
