肺部磨玻璃结节单孔胸腔镜术前CT引导下贝朗胶联合亚甲蓝定位技术的应用

Application of CT-guided localization with medical glue and coeruleum methylenum injection in treatment of ground-glass opacity with uniportal video-assisted thoracoscope surgery

摘要:

探讨CT引导下注射亚甲蓝联合贝朗胶行肺部磨玻璃结节（GGO）术前定位在单孔胸腔镜手术中的应用。方法 回顾分析该院2015年6月－2017年10月收治的30例GGO患者的临床资料，术前采用CT引导下注射亚甲蓝联合贝朗胶定位，观察操作的安全性和有效性。结果 所有患者均顺利定位，定位时间15～32?min，平均时间（20.1±4.7）min；气胸1例，血气胸1例，无咯血及胸膜反应等严重并发症。28例患者行楔形切除，自置入胸腔镜至取出楔形标本，共用时5～22?min，平均（20.0±4.5）min，2例患者因楔形标本无法确认肿瘤而直接行肺段切除术。所有患者无中转开胸。结论 该项操作简便快捷，并发症少，大大缩短了术中探查的时间、减少了切除范围过大、误切及中转开胸的概率，值得进一步推广应用。

Abstract:

To evaluate the clinical value of CT-guided localization with medical glue and coeruleum methylenum injection in treatment of ground-glass opacity (GGO) and pulmonary nodules with uniportal video-assisted thoracoscope surgery (uni-VATS). Methods Retrospective observational data for 30 patients who underwent CT-guided localization with medical glue and coeruleum methylenum injection in department of thoracic surgery from June 2015 till now. Results CT-guided localization with medical glue and coeruleum methylenum was successful in all the patients (100.0%). The time of puncturing was (20.1?±?4.7) min. There were 1 case of pneumothorax, 1 case of hematopneumothorax. None of them had Hemoptysis or pleural reaction. Conclusions CT-guided localization with medical glue and coeruleum methylenum is safe, simple and accuracy. It is a viable technique for GGO localization.
