TCRP 术后LNG-IUS 放置对子宫内膜息肉合并中度贫血患者内膜厚度、复发率及并发症的影响

Influence of LNG-IUS placement on endometrial thickness,recurrence rate and complications of patients with endometrial polyps and moderate anemia after TCRP

摘要:

目的 探讨宫腔镜息肉电切术（TCRP）后左炔诺孕酮宫内缓释系统（LNG-IUS）放置对子宫 内膜息肉合并中度贫血患者内膜厚度、复发率及并发症的影响。方法 研究对象选取该院2014 年1 月-2016 年1 月收治子宫内膜息肉合并中度贫血患者共140 例，以随机数字表法分为对照组（70 例）和观察组（70 例）， 分别给予TCRP 术单用和在此基础上术后加用LNG-IUS 放置；比较两组患者治疗前后子宫内膜厚度、血红 蛋白水平、画报失血评估表（PBAC）评分、复发率及并发症发生率等。结果 观察组患者治疗后子宫内膜 厚度明显少于对照组和治疗前（P <0.05）；观察组患者治疗后血红蛋白水平和PBAC 评分均明显优于对照组 和治疗前（P <0.05）；观察组患者随访复发率明显低于对照组（P <0.05）；同时两组患者并发症发生率比较 差异无统计学意义（P >0.05）。结论 TCRP 术后LNG-IUS 放置治疗子宫内膜息肉合并中度贫血可有效降 低子宫内膜厚度，减少子宫出血量，避免远期复发，且未增加相关并发症发生概率。

Abstract:

Objective To investigate the influence of LNG-IUS placement on endometrial thickness, recurrence rate and complications of patients with endometrial polyps and moderate anemia after TCRP. Methods 140 patients with endometrial polyps and moderate anemia were chosen from January 2014 to January 2016 and randomly divided into control group (70 patients) with TCRP used alone and observation group (70 patients) with LNG-IUS placement after TCRP; the endometrial thickness, hemoglobin levels and PBAC score before and after treatment, the recurrence rate and the complication incidence of both groups were compared. Results The endometrial thickness after treatment of observation group were significantly less than control group before the treatment (P < 0.05). The hemoglobin levels and PBAC score after treatment of observation group were significantly better than control group before the treatment (P < 0.05). The recurrence rate of observation group were significantly lower than control group (P < 0.05). There was no significant difference in the complication incidence between the two groups (P > 0.05). Conclusion LNG-IUS placement in treatment of patients with endometrial polyps and moderate anemia after TCRP can efficiently reduce the endometrial thickness, decrease the uterine bleeding amount, prevent the longterm recurrence and not increase the complications incidence.
