Clinical application of innovative intake duct driven from nasobiliary tube via nose*

摘要:

目的??探讨自主研发的鼻胆引流管经鼻导出的摄取管对鼻胆管口鼻转换的成功率和操作时间的影响。方法?将该院内镜下逆行胰胆管造影（ERCP）检查下的118例行鼻胆管引流术（ENBD）的患者随机分为磁铁组和导丝组，磁铁组采用自创的鼻胆引流管经鼻导出的牵引导管和鼻导管，将装有高性磁铁的一端经口腔送至口咽部，顺利导出鼻胆管，导丝组采用导丝从咽后壁牵引拉出胃管或导尿管再将鼻胆管固定。结果?磁铁组的用时短，一次成功率明显高于导丝组，咽部刺激减小、并发症减少。结论?采用自制鼻胆管导出的磁性导管一次成功率高，简单易行，耗时较短，患者反应轻，避免口腔黏膜的损伤，从而减少患者的痛苦，提高手术效率。

Abstract:

Objective?To investigate whether an innovative intake duct driven from nasobiliary tube via nose improved the success rate of oronasal conversion and shorten the operation time.?Methods?118 patients with routine nose bile duct drainage under ERCP examination were randomly divided into magnetic group and thread group. Patients in magnetic group were using a self-made nasal biliary drainage tube derived via nasal catheter traction and nasal catheter, one end of the magnet will be equipped with high performance through mouth to mouth pharynx, smooth delivery nose bile duct. While patients in thread group adopts godet from pharynx posterior wall drawing pulled stomach tube or catheter again, then fix the nose bile duct.?Results?The success rate was significantly higher in magnetic group than thread group, with less stimulation of pharynx, and less complications.?Conclusion?Self-made intake duct derived magnetic catheter was simple and less stimulation, avoid oral mucosa damage, reduced the suffering of patients, shortened operation time and improved the operation efficiency with high success rate.
