经膝关节镜引导小切口手术治疗胫骨平台骨折疗效及对膝关节功能和生活质量的影响

Efficacy and impact on knee joint function recovery and life quality of small incision surgery guided by knee arthroscopy for patients with fracture of tibial plateau

摘要:

目的 探讨经膝关节镜引导小切口手术治疗胫骨平台骨折（FTP）疗效及对膝关节功能和生活质量的影响。方法 回顾性分析83例FTP患者并分为对照组和观察组，分别给予传统切开内固定术治疗和经膝关节镜引导小切口手术内固定治疗。比较两组患者手术状况、并发症发生状况、膝关节功能恢复及生活质量。 结果 与对照组相比较，观察组患者手术时间明显延长，下床活动时间明显缩短，术中出血量与术后引流量均明显减少（均P <0.05）。观察组术后并发症发生率明显低于对照组患者，4.3% vs 27.0%，P <0.05。观察组患者膝关节功能恢复状况明显优于对照组，其优良率明显高于对照组患者，91.3% vs 70.3%，P <0.05。治疗后，两组患者的生活质量项目评分均明显升高，观察组躯体功能、躯体角色、机体疼痛、总的健康和活力等项目评分均明显高于对照组（均P <0.05）。结论 经膝关节镜引导小切口手术治疗FTP创少小、疗效可靠，可以促进膝关节功能恢复和提高生活质量。

Abstract:

Objective To analyze the efficacy and impact on knee joint function recovery and life quality of small incision surgery guided by knee arthroscopy for patients with fracture of tibial plateau (FTP). Methods We collected and retrospectively analyzed 83 FTP cases and divided them into the control group receiving traditional surgical internal fixation and the observation group receiving small incision surgery guided by knee arthroscopy. Surgery status, complications, knee joint function recovery and life quality of 2 groups were observed and compared. Results Compared to the control group, the surgery time extended, activity time decreased, the bleeding volume and the drainage decreased, P < 0.05. The complication rate of was significantly lower than that of the control group (4.3% vs 27.0%), P < 0.05. The excellent and good rate of the function of knee joint in the observation group was 90.3%, which was significantly higher than that in the control group (70.3%), P < 0.05. knee joint function recovery of the observation group was significantly superior to the control group, P < 0.05. The life quality of the observation group was significantly higher than that of the control group, P < 0.05. Conclusion Treatments of small incision surgery guided by knee arthroscopy is less traumatic, effective for improving the knee joint function recovery and life quality, and decreasing the incidences of complications in treating FTP.
